What we need is the following to enroll each client
1. Client's application
2. Signed physicians form completed by doctor
3. Complete Assessment with diagnosis
4. ISP Form (face sheet) and discharge information
5. TB test results

Please fill out the application completely. All completed forms need to be fax and or emailed to the following. Fax to 336.625.0527 or emailed to rfh@centurylink.net
Physican's Order Form
Randolph Fellowship Home must recieve physican forms for all consumers before they can enter the homes. The physican forms must be signed by a doctor with any over counter medication allowed by consumer as well as all prescribed medication. Consumers are not allowed to take any type of benzos or narcotics while consumers in our homes. Random drug screens are given frequently.
